Program Overview
The SOAR Family Model: A Home-Based Approach to Supportive Living
The SOAR Family Model creates genuine home environments where adults with intellectual, developmental, and physical disabilities can thrive with the perfect balance of independence and support. As a Family Model provider, you’ll welcome individuals into your home as valued family members, supporting their growth while building rewarding relationships. Unlike traditional care facilities, the Family Model offers residents the comfort and stability of a real home, personalized attention, and meaningful community integration—all while providing caregivers like you with a fulfilling opportunity to make a lasting difference.
The Rewards of Being a Family Model Provider
-
Meaningful Relationships
Build genuine connections and witness the growth and achievements of individuals you support. Many of our providers describe the relationships formed as the most rewarding aspect of the program.
-
Financial Support
Caregivers can earn between $1,000 and $16,000 per month depending on residents' support needs. Additional compensation includes room and board payments (typically 70% of residents' SSI/SSDI benefits) and potential funding for community activities.
-
Professional Support
You'll never be alone in this journey. SOAR Tennessee provides comprehensive training, ongoing guidance, regular check-ins, respite options, and a supportive community of fellow providers.
Understanding the Family Model Approach
-
Individualized Support Levels
The Family Model offers six different support levels based on residents' needs, from light assistance with daily activities to comprehensive support for complex needs. This tiered system ensures that both caregivers and residents receive appropriate resources.
-
Home Environment
Each Family Model home welcomes up to three residents, creating a balance between personalized attention and social connection. You'll help create a comfortable, supportive home where residents can develop independence and skills.
-
Community Integration
Beyond daily care, you'll support residents in building connections through SOAR community events and activities in the broader community. Additional funding is available for Community Participation Support for up to 30 hours weekly.

"Living here has changed my life. I have my own space and independence, but I also have people who care about me and help me reach my goals. It feels like home."
